HREP is an innovative venture launched through ADPH Office of HIV Prevention and Care to utilize licensed social workers to locate HIV positive patients that have been out of care for one (1) to ten (10) years and re-engage them back into HIV medical care and support services, decrease number of people living with HIV that are not in care and increase receipt of care, retention in care and viral suppression, thus helping to end the HIV epidemic.
