Pre-Licensure Application

A Pre-Licensure Application is required for all new health care facilities that are required to submit building plans and specifications for review. This application must be submitted and approved before plans will be reviewed by Technical Services.

This application is not required for additions or alterations to licensed facilities. Additional information regarding architectural review can be found at Facilities Management and Technical Services.
